1. Original Cash Register Receipt dated between 7/12/10 and 11/30/11 (up to $4.49 value) with price circled. Photocopies are not accepted.
2. 1 UPC symbol from Kellogg’s FiberPlus™ cereal. One request per flavor of specially marked product per name, address or household.
3. Request must be postmarked by 11/30/11 and received by 12/10/11.
